Thank you for your part in partnering with the Verizon Innovative Learning Program for Minority Males at Fisk University (VIL MM).  We are gearing up to collect the pre-study survey data for the Verizon Innovative Learning Program.

 

Attached, please find instructions for administering the survey, student roster, consent form, and student login cards for the Verizon@FISK Science Club at your school.  The goal is to have the surveys completed by Tuesday, October 9th.

 

Prior to administering the survey, please assign each student a login ID as listed in the roster and associated login card.  Then, u sing the following link and Log in code to take the survey:

Please ensure that all students who participate in the survey have a signed consent form prior to participating in our evaluation activities. This is completely voluntary.  If there are any students who choose to opt out, do not have them take the survey.
